网友采纳 insist是否使用should是要根据具体含义的当insist表示“坚持，坚决要求，执意”的时候，后面要跟虚拟语气，所以要加should当表示坚持说，固执己见的时候，不用虚拟语气所以这个句子其实insist引导了两部分从句insistedthathe_was____badlywounded坚持说他伤情严重这个时候没有“要求执意”的含义只是表达观点and（insisted）thathe_beoperatedon___atonce.坚决要求他马上做手术这个时候是你说到的用法should可以省略
